Information on the processing of personal data
In accordance with Art. 13 sec. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation – GDPR), we inform you that:
1. The Personal Data Controller is Maxus Sp. z o. o., with its registered office at ul. 3 Maja 64/66N, 93–408 Łódź.
2. The Controller has appointed a Data Protection Officer, who can be contacted via e–mail: wojtowicz@kancelariawojtowicz.pl
3. Personal data will be processed on the basis of:
- a) Art. 6 sec. 1 lit. a of the Regulation – consent to the processing of personal data collected for the purpose of preparing an offer;
- b) Art. 6 sec. 1 lit. b of the Regulation – for the purpose of performing concluded contracts, customer service, and recovering outstanding amounts from debtors;
- c) Art. 6 sec. 1 lit. c of the Regulation – for the purpose of fulfilling legal obligations incumbent on the controller, including hiring employees, performing activities related to HR services, enabling the use of reliefs provided by PFRON, managing invoices and contact data from invoices;
- d) Art. 6 sec. 1 lit. f of the Regulation – for the purpose of ensuring security on the premises of the facility, by recording the image of persons staying there via video monitoring.
4. Recipients of personal data may be entities entitled to obtain them on the basis of legal provisions or concluded agreements.
5. Personal data will be processed for the period necessary to achieve the goals indicated in point 3, and after that time, for the period required by law. Video monitoring recordings will be stored for a period of 30 days.
6. You have the right to access your data, rectify them, delete them, or limit their processing, as well as the right to object. If the processing of personal data takes place on the basis of consent, you have the right to withdraw it at any time, without affecting the lawfulness of the processing performed on the basis of consent before its withdrawal.
7. If it is found that personal data are not processed in accordance with the requirements of the Regulation, you have the right to lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office.
8. Providing personal data is voluntary; however, if they are not provided, we will not be able to provide services to you.
9. You are not subject to automated decision-making, including profiling.
